Pain in the lower abdomen is a typical sign of diseases of the gastrointestinal tract or genitourinary system. Therefore, most people turn to a doctor of the appropriate specialization. But when the results of the examination indicate the normal state of the internal organs, both patients and doctors are perplexed: how can this be? In such situations, it is necessary to pay attention to the spine: in 10-20% of cases, the “culprit” of abdominal pain is a pathology of the musculoskeletal system.

Causes of abdominal pain in men

Specific to men are conditions in which patients feel girdle pain in the abdomen and lower back:

  • Inflammation of the prostate - accompanied by pain, frequent difficulty urinating, and spermatogenesis disorders.
  • Orchiepididymitis is an inflammation of the epididymis and testicles that results from trauma to the reproductive organs, genital tract infections, or surgery.

  • Inguinal hernia - in addition to pain, fever and chills rise, the patient loses consciousness. In this condition, urgent hospitalization is necessary.
  • Infectious diseases of the reproductive system, including sexually transmitted diseases, are transmitted through sexual contact and are provoked by fungi and bacterial infections. In this case, pain occurs in the lower abdomen, groin, and discharge appears. Without treatment, the disease becomes chronic.
  • Colitis - most often occurs against the background of bloating and increased gas formation. In the acute form, sharp, cramping pain appears in the intestines, and when it passes into the chronic stage, a constant nagging pain is felt.

What is the connection between osteochondrosis and pain in the lower abdomen?

Abdominal pain is a concomitant symptom of lumbar, thoracic (much less frequently) osteochondrosis, since in these parts there are nerve endings of the spinal cord connecting the spine with the abdominal and pelvic organs.

Over the years, intervertebral discs deteriorate: they lose their strength and elasticity. This leads to excessive mobility of the vertebrae and the formation of bone growths on their body, the appearance of pathologies in the joints, muscles and ligaments (compression of the paravertebral structures occurs). During degenerative processes in the thoracic region, in the area of ​​5-7 vertebrae, the roots responsible for the nervous supply of the intestines, gallbladder, spleen and stomach are compressed. Their constant irritation by overgrown osteophytes causes pain that mimics gastrointestinal diseases.

The mechanism of pain in the lower abdomen with lumbar osteochondrosis is based on the following compression and reflex syndromes:

  1. Radicular - manifests itself both in the early and later stages of the disease (when a protrusion or lateral intervertebral hernia has formed). As the degenerative disc moves, it compresses the nerve roots and blood vessels, thereby impairing the outflow of venous blood and the transmission of nerve impulses from the spinal cord to the brain (spasm, inflammation or paralysis of the spinal nerves occurs). This leads to pain in certain parts of the body. If the first three roots are pinched, a person feels pain in the lower back, front and inner thighs, lower abdomen and groin area.
  2. Lumboischialgia (sciatica) is a complex of symptoms that occurs as a result of pinching of the sciatic nerve by deformed vertebrae or intervertebral hernia. The syndrome manifests itself in different ways. As a rule, the patient feels shooting pains in the back, radiating to the thigh and buttock. However, a compressed sciatic nerve can also cause spasms of the pelvic muscles: pain appears in the groin and lower abdomen.

Description of symptoms

Depending on the nature of the disease, the nature of the pain can vary significantly. Additional symptoms appear (for example, if it radiates to the back and frequent urination or bloating is noted - the causes of abdominal disease may be different), which completely gives a picture for making a diagnosis.

Bloating

Bloating is a reaction of the body to certain processes occurring in it. It manifests itself as a result of the accumulation of a large amount of gases in the intestines. They do not go outside, but accumulate inside, expanding in volume.

As a result, the tissue begins to stretch, causing sharp pain. It intensifies when driving and turning. The condition can be alleviated due to the release of gases.

As a rule, pain symptoms have a wave-like character, periodically intensifying with movement or with the arrival of new gases.

Sharp

Sharp pain is an unexpected strong and sharp feeling, the intensity of which can increase over time. Intensifies with active movements, for example, when walking. Exacerbation of pain can be noticed when:

  • prolonged cough,
  • sigh,
  • change in body position.

In the presence of such painful symptoms, the abdominal muscles tense. The doctor records this during the examination.

Important! Acute and severe radiating pain indicates the possibility of diseases that pose a danger to human health and life.

Such symptoms require urgent treatment, and sometimes hospitalization and immediate medical intervention. Most often, such symptoms are inherent in the following diseases:

  • ulcer,
  • splenic rupture,
  • ectopic pregnancy,
  • inflammation of the intestinal diverticulum,
  • acute cholecystitis,
  • enterocolitis,
  • appendicitis.

Aching

Aching pain is most often chronic and lasts from several days to several months. This pain is dull, at times it can be felt stronger, and sometimes it subsides and becomes almost unnoticeable.

Sometimes aching pain is accompanied by such symptoms as:

  • vomit,
  • sweat on the forehead,
  • be sick,
  • chest pain and shortness of breath,
  • bloating and swelling.

The presence of such sensations is symptomatic of various functional disorders. These include irritable bowel syndrome.

In some cases, in children, such pain can cause reflux, esophagitis, diverticulitis, peptic ulcer, colitis, and so on.

Pulling

If your lower abdomen is tight, then most often it seems that not only a specific part of your body is hurting, but also nearby neighboring organs. Such pain irritates, drives you crazy, and leaves you bedridden. Most often, it is characteristic of women during the premenstrual and menstrual periods.

But there are other reasons for such sensations. Nagging pain is inherent in chronic inflammatory and tumor processes. Accompanies a disease such as parenchymatosis.

The second cause of nagging pain can be a sprain of the ligaments that support the organs in the pelvis. This happens due to the heavy load on the ligamentous apparatus. For example, due to fetal enlargement during pregnancy, heavy lifting.

The third reason is the adhesive process that occurs in the pelvis. Adhesions can appear both as a result of serious illnesses and after operations.

Shingles pain can be both chronic and acute. If such symptoms are accompanied by elevated body temperature and the urge to vomit, an urgent visit to a specialist is necessary.

Possible causes of discomfort:

  • pancreatitis,
  • cholelithiasis,
  • myocardial infarction,
  • radicular syndrome,
  • stomach ulcer,
  • gynecological diseases,
  • aortic aneurysm,
  • diseases of the urinary system.

Constant

Constant abdominal pain is characterized by a relatively stable course. Patients most often describe the sensation as a burning sensation in the stomach or a feeling of hunger. This is a consequence of serious inflammatory processes in the abdominal organs or a harbinger of ulcerative lesions.

Why do lower back pain and abdominal pain depend on each other? The reasons may be:

  • dysbacteriosis,
  • hepatitis A, B, C,
  • reactive pancreatitis,
  • mononucleosis,
  • gastritis,
  • appendicitis,
  • peptic ulcer,
  • pyelonephritis,
  • chronic gastrointestinal infections,
  • gynecological diseases, such as adnexitis,
  • urolithiasis disease.

Very often, abdominal pain is accompanied by pain in the lower back. There is a natural explanation for this.

Reference. All vital organs of the digestive tract and genitourinary system are located between the lower back and the abdominal cavity. At the moment when one of the organs is affected and inflamed, the pain goes not only to the stomach, but also to the lower back.   • worries constantly;
  • worsens when bending over, changing body position, walking, deep breathing and coughing;

During destructive processes in the bone and cartilaginous tissues of the spine, pain in the abdomen is accompanied by:

  • heartburn, nausea and even vomiting;
  • feeling of heaviness in the hypochondrium;
  • bowel disorder (constipation, diarrhea);
  • excessive gas formation in the intestines.

With osteochondrosis, the clinical manifestation of pain in the lower abdomen is often perceived as signs of appendicitis, colitis, cholecystitis or adnexitis, and ailments of the genitourinary system. However, with a disease of the internal organs, the patient also develops other symptoms (see table)

  • if the pain does not stop within 60 minutes;
  • it increases with coughing and movement;

Organ pathology or osteochondrosis: how to find out why your stomach hurts?

It is possible to identify the cause of pain in the lower abdomen only after the patient undergoes the following types of examination:

  1. Ultrasound of the abdominal and pelvic organs is prescribed to confirm (refute) their pathology.
  2. X-ray of the spine (in lateral and direct projections) - allows you to assess the condition of the vertebrae: the degree of their displacement, the presence of osteophytes.
  3. CT and MRI are the most informative methods for studying tissue. They make it possible to examine the intervertebral discs (to clarify their edges) and the ligamentous apparatus, to find out the location of the infringement of the spinal roots.

The relationship between pain syndrome and osteochondrosis is confirmed if the diagnostic results show:

  • normal condition of internal organs;
  • reducing disk density;
  • the presence of osteophytes on the vertebrae;
  • destruction of cartilage inside the facet joints;
  • formation of protrusion, intervertebral hernia;
  • spinal deformities (scoliosis, lordosis, kyphosis).

In severe cases, making an accurate diagnosis may require consultation with highly specialized specialists (gynecologist, infectious disease specialist, neurologist) and additional examination, which includes:

  • urine blood tests (general and extended);
  • bacteriological culture of stool;
  • endoscopic examination of the stomach, intestines;
  • coprogram.

Why does the lower back and lower abdomen hurt?

This pain syndrome can be studied in three projections, in which the causes may be:

Lower back and abdominal pain in women

There are many reasons for diseases of this kind in women, and they are mainly associated with gynecological problems:

These are pains before menstruation (premenstrual syndrome) and during menstruation. Such pain is associated with changes in the uterus:

  • Before the start of the cycle, it increases in size and becomes tense, and the lower abdomen ache. A woman may become irritable and nervous during this period.
  • During menstruation, the pain may become cramping in nature, which is associated with active contractions of the uterus

  • due to inflammation or pathologies of the uterus (endometriosis, uterine flexion, etc.)

Pulling in the lower abdomen from cystic and tumor formations (benign and malignant):

Pain in pregnant women

If a pregnant woman feels pain in the lower abdomen and back:

When men have lower abdominal pain

Men also experience pain in the lower back and lower abdomen. The reason for this may be:

  • Prostatitis
  • Inguinal hernia
  • Appendicitis

  • Ulcerative colitis
  • Bacteriological (in particular, sexually transmitted diseases)
  • Intestinal inflammation

  4. Intestinal infections cause severe upset, vomiting, traces of mucus and blood are visible in the stool
  5. Bacteriological sexually transmitted diseases usually begin with an acute form: the temperature may rise, pain and discharge may occur in the genital area. If left untreated, they will become chronic with dull aching pain in the groin, lower abdomen and lower back

What to do if you have pain

If there is acute pain in the lower abdomen and lower back, combined with other symptoms, you need to call an ambulance.

What may be the signs:

  • The pain does not go away for a long time and intensifies with the slightest movement
  • Associated with vomiting, high fever, blood in stool or black stool
  • There are signs of intestinal obstruction (hard, bloated abdomen, lack of stool)

You cannot do the following:

  • Before the doctor arrives, give some medications (except no-shpa) and drink
  • Do cleansing enemas
  • Apply warm compresses

Chronic pain syndrome of the abdomen and lower back

However, with a chronic pain symptom, it is often not possible to find out why the lower back and lower abdomen hurt:

Ultrasound, MRI of the pelvis, and complex biochemical tests do not reveal any diseases, although women complain of obvious signs of adnexitis, and men of prostatitis.

In this case, the neuralgic nature of the syndrome is suspected and referred to a neurologist.

The reason for such false symptoms is that there are a large number of nerve controllers and autonomic plexuses in the pelvic area. The causes of damage to the nerve ganglia are:

  • Injuries
  • Intoxication
  • Herpes
  • Worm infestations

It also happens that the cause of pain is outside the autonomic nervous system, namely: it is located in the brain.

Thus, an attack of epilepsy can cause so-called sensory paroxysm, which manifests itself in the form of chronic pain syndrome.

Diseases of the musculoskeletal system and lower back pain

The cause of nagging pain in the lower back and lower abdomen can also be more prosaic reasons associated with diseases of our musculoskeletal system:

  • Muscle strain (especially after abdominal exercises)
  • Osteochondrosis
  • Intervertebral lumbosacral hernia The cause of pain is the involvement of the sciatic nerve in the process, and the pain can even radiate to the groin and leg
  • Scoliosis of the lumbosacral region. In this case, there is a displacement of not only the spine relative to the frontal axis, but also internal organs, which affects their work and can cause all kinds of dysfunctions, causing pain in the lower abdomen with their projection to the lower back

However, the presence of such diseases does not directly mean that the lower abdomen and lower back hurt precisely because of them:

The complex structure of the female body, undergoing monthly cyclic changes, can signal them in the form of various unpleasant sensations. This may include dizziness, sometimes even fainting, fluctuations in blood pressure, pain in the mammary glands, lower back and abdomen. However, if the lower abdomen and lower back hurt persistently, then you should not rush to attribute everything to physiology. It is necessary to find out the cause of the pain so as not to miss a serious pathology, which also gives similar symptoms.

Why does the lower abdomen and lower back hurt during menstruation?

During menstruation, the lower abdomen hurts quite often. Why does this happen and what causes these conditions in women? But the fact is that the endometrium, which has served its term, must be rejected and safely leave to make way for a new layer. Such rejection is accompanied by various processes leading to changes in the endometrium.

The vessels feeding it begin to spasm and become clogged with microthrombi, as a result of which ischemia develops in the endometrial layer of the uterus. In addition, the appearance of pain is facilitated by the accumulation of tissue breakdown products, which is characteristic of this period. These pains will be physiological, but they are not so intense as to reduce the ability to work. Rather, it is not so much pain as a pulling sensation corresponding to the period of desquamation (squamation) of the endometrium.

It should be noted that the sensitivity threshold is also different for everyone, that is, with a low threshold, even in the absence of pathology, a woman will experience pain, which will knock her out of her usual rut during her period.


Other pains, called algodismenorrhea, which can be primary or secondary, are caused by pathological changes. Primary algodysmenorrhea occurs simultaneously with menarche and is caused by the peculiarities of the anatomical location of the uterus or a violation of the ratio of sex hormones and prostaglandins. An excess of the latter leads to increased contraction of the muscular layer of the uterus, which can be accompanied not only by aching, pulling or cramping pain in the lower abdomen, but also give symptoms such as headache, nausea, vomiting, tachycardia, and dizziness.

Secondary algodismenorrhea is caused by:

  • inflammation of any etiology, localized in the pelvic organs;
  • the presence of intrauterine contraceptives;
  • endometriosis;
  • hyperplastic conditions of the endometrium;
  • tumors;
  • adhesive process in the pelvis.

It happens that menstruation has already ended, but the pain does not go away and continues to bother the woman. You should especially pay attention to it, because if there is pain in the lower abdomen after menstruation, as a rule, this indicates a pathological condition of the genital organs.

Women should know that most often the lower abdomen hurts and pulls, and brown discharge continues to occur before and/or after menstruation due to endometriosis, a rather serious and insidious disease.

Low back pain due to endometriosis and uterine fibroids

The third place among all gynecological diseases belongs to endometriosis, a pathological process characterized by the growth in the genital and other organs of tissue similar to the endometrium in morphological and functional characteristics.

Perhaps, only cervical endometriosis is painless, which manifests itself only as scanty bleeding before and after menstruation, lasting 3-4 days.

The presence of endometriosis of the uterine body, in addition to sharply painful, prolonged and heavy menstruation, leads to anemia of the body due to uterine bleeding.

A fairly common form of genital endometriosis is ovarian endometriosis, which is characterized by constant pain in the lower abdomen and lumbosacral spine, intensifying during menstruation. In addition, with this form of the disease, due to the entry of tissue and blood particles into the abdominal cavity, a pronounced adhesive process quickly develops, which will certainly cause constant pain.

Retrocervical endometriosis is particularly painful in the sacrum and lower back, radiating into the rectum and vagina. Sexual intercourse, defecation and menstruation cause a lot of trouble for a woman, since they are accompanied by pain.

Foci of endometriosis in the tubes, ovaries, and peritoneum, which result in a pronounced adhesive process, lead to the development of chronic pelvic pain syndrome in women. In addition to endometriosis, this syndrome can be a consequence of tumors, benign and malignant. However, if the latter require immediate surgical intervention, then benign fibroids, as a rule, exist for years and give constant pain in the abdomen, sacrum and lower back.

A hormone-dependent tumor that develops from muscle tissue, fibroids, often causes painful and heavy periods. Constant aching nagging pain is most often caused by stretching of the peritoneum and pressure of myomatous nodes on the nerve plexuses of the pelvis, which occurs when the nodes located under the peritoneum grow.

Sometimes painful sensations cause dystrophic and necrotic changes in the uterus affected by nodes. Submucosal localization of fibroids is characterized by cramping pain during menstruation.

Myomatous nodes can impair the functionality of the rectum and bladder, which will also be accompanied by pain in the lower abdomen.

Chronic pelvic pain syndrome significantly affects a woman’s psyche, causing depression, neuroses, and apathy, and therefore requires targeted combined treatment. In this case, pain associated with pathology of the reproductive system should be differentiated from diseases of the kidneys, intestines, and bladder.

Pain on the right, left or both sides...

It's difficult with women. There is constant pain in the lower abdomen on the right. In men, as a rule, appendicitis is suspected; in women, pain can also be caused by an inflamed appendix, and the appendages may also bother them. Therefore, differentiating these diseases is a gynecologist’s task, by the way, not always simple, since appendicitis is quite difficult to diagnose and can manifest itself as painful sensations in any part of the gastrointestinal tract, including in the epigastric region, but most often the side on the right side hurts with right-sided inflammation appendages and appendicitis. Pain in the lower abdomen on the left, in general, occurs for the same reason:

  • appendicitis;
  • adnexitis;
  • colitis.

Women complain that the lower abdomen and ovaries hurt due to diseases such as:

  • adhesive disease in the pelvis;
  • ovarian cysts that put pressure on the surrounding tissues, where a special place belongs to the endometrioid (“chocolate”) cyst, which is localized first in one ovary, then quickly spreads to the other and is accompanied by the development of a pronounced adhesive process with the formation of a single conglomerate consisting of the uterus and appendages;
  • tuberculosis;
  • endometriosis, localized in the ovaries;
  • adhesions in the ovaries as a result of inflammation.

And yet, the most common cause of pain is inflammatory diseases of the genital organs of specific and nonspecific etiology. The first group includes processes caused by gonococci, chlamydia, mycoplasma, trichomonas, and viruses.

The second group consists of diseases caused by the pathogenic influence of staphylococci, Pseudomonas aeruginosa, streptococci, and Escherichia coli. Chronic inflammatory processes can manifest as frequent exacerbations and cause cicatricial adhesive changes in the organs of the urogenital tract, which will also be accompanied by chronic pelvic pain.

Inflammation of the mucous membrane of the cervix (cervicitis), the causative agent of which is a bacterial or viral infection, is greatly promoted by trauma to the cervix during childbirth, artificial termination of pregnancy, erosion, colpitis and other diseases of the urogenital tract. With cervicitis, nagging pain in the lower abdomen and lower back in the acute period is accompanied by mucopurulent leucorrhoea, but in the chronic stage the discharge becomes white, and the painful sensations almost subside. By the way, the cause of white discharge can be both dysbiosis and an increased concentration of progesterone in phase II of the menstrual cycle, so leucorrhoea does not serve as a clear diagnostic sign of cervicitis.

Inflammation of the uterine mucosa (endometritis) can be a consequence of any intrauterine interventions (childbirth, complicated abortions, diagnostic curettage, etc.).
