Omez: how it works and what it helps with

Omez - what is it and what is it for?

The common medicine Omez - the composition of which is simple and consists of omeprazole - is popular among doctors and is used in the treatment of diseases of the gastrointestinal tract. The drug normalizes the secretion of gastric juice, fights high acidity, reduces the increased impact of external factors on stomach receptors, which force the mechanism to produce basal secretions outside of meals. The drug contains an active substance supplemented with auxiliary ingredients. The medicine is available in 3 types of capsules with 10, 20 and 40 mg of omeprazole, varying in price.

What is it prescribed for? Omez has a wide spectrum of action. This allows it to be prescribed for the following diseases:

  • stomach and duodenal ulcers;
  • damage to the lower part of the esophagus, caused by periodic release of stomach or intestinal contents into the gastrointestinal tract;
  • erosive or ulcerative lesions of the duodenum or stomach;
  • Gastroesophageal reflux disease (GERD);
  • gastrinoma;
  • reflux esophagitis;
  • Zollinger-Ellison syndrome;
  • erosion of the upper part of the stomach, which develops against the background of cirrhosis;
  • the development of an infection that destroys beneficial bacteria on the gastric mucosa;
  • gastritis;
  • heartburn.

Omez is often taken for preventive purposes. For example, in order to exclude the possibility of developing a recurrence of a stomach or duodenal ulcer. Also, the drug can be prescribed to the patient 2 - 3 hours before surgery in order to prevent the possibility of acidic stomach secretions entering the respiratory system.

Pharmacodynamics and pharmacokinetics

The use of the medicine internally leads to a rapid development of the effect: it manifests itself within 1 hour. Then Omez tablets are valid for 24 hours.

After the medication is stopped, the secretory activity of the exocrine glands of the stomach is restored after about 3-5 days. Omeprazole is characterized by rapid absorption from the intestine. Since the active substance in the drug is contained in acid-resistant granules, they dissolve only in the intestines. In plasma, the highest concentration is observed after 30-60 minutes. The level of bioavailability of omeprazole is 40%. The binding of the drug to plasma proteins is 90%. Metabilized in the liver. Excreted through the kidneys.

When administered intravenously, dose-dependent inhibition of gastric secretion is observed. The half-life of omeprazole after intravenous administration is approximately 40 minutes.

Composition and release form

Contains two active ingredients: omeprazole (20 milligrams) and domperidone (30 milligrams). Both components are made in the form of special granules that dissolve in the intestines. Domperidone is contained in a sustained release form. This feature helps prolong the action of the chemical compound and reduce the frequency of taking capsules.

Package

Additional substances: talc, mannitol, milk sugar, sucrose, sodium dodecyl sulfate, hypromellose, sodium hydrogen phosphate, colloidal silica. There are also auxiliary components that form the capsule and additional elements of the dosage form.

What does it help with? Indications for use of Omez capsules

The remedy is most often recommended for gastritis and peptic ulcers, both for treatment and for the prevention of exacerbations. What else does the drug help with? Indications include various disorders of the stomach and esophagus caused by increased acidity:

  • GERD, or gastroesophageal reflux disease, including complicated by reflux esophagitis;
  • chronic atrophic gastritis with high acidity;
  • erosive gastritis;
  • mixed gastritis;
  • duodenitis;
  • erosion, duodenal ulcer;
  • idiopathic increase in acidity in the stomach (idiopathic hypersecretion);
  • peptic ulcer, ulcer due to stress;
  • ulcer, gastritis, erosion caused by taking anti-inflammatory drugs;
  • systemic mastocytosis.

Omez tablets also help against infection - they help eliminate Helicobacter pylori, therefore they are prescribed in complex therapy along with antibiotics. The medicine is also given to patients with chronic pancreatitis - it has been proven that a course of treatment leads to an improvement in the functional state of the pancreas.

Omez is indicated for children in the following cases:

  • The presence of a duodenal ulcer, which arose as a result of exposure to Helicobacter. Suitable for children weighing over 20 kg. and over 4 years old.
  • The presence of gastroesophageal reflux in young patients over 2 years of age. Body weight should be from 20 kg.

Data on the effective and safe use of Omez in children with other ailments have not been established.

Contraindications ⛔

Omez, like any other drugs, is characterized by the presence of contraindications. Despite the fact that there are few direct contraindications to the use of omez, the peculiarities of its pharmacological action force one to take certain precautions. They protect the patient from the development of side effects. In general, when used correctly, it is well tolerated, but in some patient groups its use is prohibited.

Omez is contraindicated for use in the following cases:

  • individual hypersensitivity to omeprazole;
  • children's age (up to 4 years – for capsules and up to 18 years – for intravenous administration);
  • combined treatment with nelfinavir, atazanavir.

Thus, the drug is contraindicated for use in young children; for children over 4 years of age, dose adjustment is necessary. Nelfinavir and atazanavir are antiretroviral drugs that are used to fight HIV (human immunodeficiency virus). When using these drugs, omez cannot be used, as it reduces the concentration of antiretroviral drugs in the blood, which is very dangerous for HIV patients.

Omez has restrictions for use in the following cases:

  • stomach cancer;
  • liver failure;
  • renal failure;
  • osteoporosis;
  • combined use of clopidogrel.

The presence of restrictions does not completely exclude the possibility of using omez, but warns against rash use. In case of stomach cancer, omez can hide the symptoms and complicate diagnosis; if the kidneys or liver are damaged, its removal from the body slows down. Use for osteoporosis increases bone fragility and can lead to fractures. Clopidogrel is a drug that reduces the formation of blood clots. Its combination with omez in practice can lead to unpredictable consequences for the cardiovascular system, so it should be avoided.

Hypersensitivity to omeprazole

One of the most important contraindications to the use of Omez is hypersensitivity to omeprazole or other components of the drug. It manifests itself in the form of various allergic reactions. Allergy to omeprazole is quite rare, but the patient may experience it. Allergy symptoms are quite unpleasant and can even be fatal.

Hypersensitivity to omeprazole when taking the drug may manifest itself as follows:

  • the appearance of skin rashes;
  • redness and swelling of the facial skin;
  • nausea, vomiting, increased stomach pain;
  • difficulty breathing;
  • decreased blood pressure;
  • loss of consciousness.

Description
It is difficult to compare these two identical acid-controlling drugs. Based on one active substance (omeprazole), they are similar in absolutely all points of the instructions. The only difference is in the price (the price of Omeprazole is lower than that of Omez), the manufacturer, and the concentration of the active substance (in Omeprazole it is higher, unlike Omez, where additional components are added that mitigate unwanted responses of the body).

How to take Omez? Instructions for use of capsules + dosage

Regardless of the diagnosis, before you start taking Omez, you should read the instructions, which describe in detail how and why to take Omez. Instructions for the use and prescription of Omez are described in detail, here is some dosage of taking the drug for different diagnoses:

  • For duodenal ulcers, Omez is prescribed 1 tablet every day. The duration of treatment in this case is from 14 to 28 days. If the disease worsens, the dosage can be increased to 2 tablets per day.
  • In case of diagnosis such as erosion or ulcer, violation of the integrity of the walls of the stomach or intestines, provoked by taking NSAIDs (non-steroidal anti-inflammatory drugs), the dose of Omez remains unchanged (1 - 2 tablets), but the course of treatment lasts 1.5 - 2 month.
  • If you are diagnosed with reflux esophagitis, or if it is necessary to exclude relapses of this disease, the doctor may prescribe a course of taking Omez for 5 to 6 months.
  • The diagnosis of gastrinoma requires an individual approach to dosage selection. This is due to the different level of gastric secretion in each individual case. At the initial stage of treatment, it is recommended to take 3 tablets per day, and subsequently the dose can be increased from 4 to 6 tablets twice a day.

When prescribing Omez for prophylactic purposes to patients with ulcers, the dose is 10 mg. once a day.

In some cases, the dosage requires specific administration measures:

  • liver failure (dose 10-20 mg.);
  • renal dysfunction.

For elderly patients, dosage adjustment is not carried out.

Intravenous administration of Omez has its own characteristics. In the case of gastroesophageal reflux disease, as well as peptic ulcers, the drug is prescribed in a dosage of 40 mg. as an infusion. If the patient suffers from Zollinger-Ellison syndrome, the initial dosage of the drug is 60 mg. per day. Significantly larger doses are often prescribed. Usually, if the daily dose of Omez exceeds 60 mg, it is usually divided into 2 infusions.

A peptic ulcer, which is complicated by internal bleeding, requires the initial use of Omez at a dose of 80 mg. per day. The rate of administration is 8 mg. at one o'clock.

If the patient suffers from liver failure, the maximum intravenous dose of Omez per day should not exceed 10-20 mg.

How to drink Omez correctly: before or after meals

The effectiveness of using a medication will largely depend on its timely administration. Antiulcer drugs, which include Omez, should be taken in the morning, half an hour before meals. This is due to the fact that at this time the active substances are best absorbed in the gastrointestinal tract and exhibit their positive properties to the maximum. If necessary, the medication can be taken two hours after a meal, when food no longer has a strong effect on the absorption of the components of the drug.

Parenteral administration of the drug does not depend on the timing of meals, since this procedure avoids the drug entering the stomach. After injection, the active substance penetrates immediately into the patient’s systemic bloodstream.
